Health Administration Emerging Leader Award Form

HAS Outstanding Administrator-Supervisor Award

The Arkansas Public Health Association's Health Administration Section is accepting nominations its Outstanding Administrator-Supervisor Award. Any member of APHA may nominate individuals for this award. The award recipient will be selected based on the strength of the nomination as judged by the criteria below.  Awards will be presented during the annual APHA Conference in April 2020.

The Outstanding Supervisor-Administrator Award recognizes front-line public health administrators and supervisors who have exhibited strong leadership in their work with both employees and the public as well as understanding and skill at improving the systems in which they work.

Nominations that do not address all questions and meet the criteria for experience and letters of support will not be considered. Answers do not have to be long but should be as specific as possible.

Has the nominee been informed of this nomination?
The nominee must be a front-line administrator or supervisor in a local, regional, or central office. Does your nominee meet that criteria?
The nominee must have served as a supervisor or administrator of a work unit for at least one year. Does your nominee meet that criteria?
At least three letters of support from people who are employees in the same area as the nominee are required to complete the nomination for this award. Do you have at least three letters of support from three other employees to attach to this nomination?
